Lungstereotacticbodyradiationtherapy(SBRT)isaneffectiveandgenerallysafetreatmentforlungcancer,butahigherrateoftoxiceffectshasbeenreportedforSBRTwithinthecentrallungzone.PatientsreceivingSBRTfortumorsabuttingtheproximalbronchialtree(PBT)oresophagus,knownas“ultracentral”tumors,mayhaveaparticularlyhighriskoftoxiceffects.Antiangiogenicagents(AAAs)havebeensuspectedtopotentiateSBRTtoxiceffectsinotherbodysites.Herein,wereportonthetoxiceffectsassociatedwithSBRTforultracentrallungtumors,withafocusonexposuretoAAAs.

Localcontrolofultracentrallungtumorsisimportantbecauseprogressionmaycauseairwayobstructionorbleeding.However,thislocationischallengingtoaddresswithsurgeryorradiofrequencyablationowingtoitsproximitytocentralairwaysandmediastinalstructures.ThefeasibilityandsafetyofnoninvasivelocaltherapysuchasSBRTisthereforeofparticularinterest.
